Reporte de Caso Clínico. Manejo Quirúrgico de Torsión testicular derecha en paciente pediátrico

Resumen
La torsión testicular es una urgencia urológica. Aunque puede ocurrir a cualquier edad, es más común en recién nacidos y adolescentes. Hay dos tipos de torsión testicular: extravaginal (más común en recién nacidos) e intravaginal (en adolescentes). Una fijación inadecuada de los testículos puede provocar la torsión del cordón espermático que contiene los conductos deferentes, lo que puede provocar un mayor infarto de la glándula. La torsión testicular puede ocurrir como consecuencia de una lesión o ejercicio, pero también puede ocurrir de manera espontánea. Se presenta con dolor y náuseas de moderados a intensos. En la exploración, el reflejo de elevación escrotal suele desaparecer. Posteriormente se revela: aumento del tamaño y ecogenicidad de los testículos, el testículo puede estar elevado y lateral en comparación con el testículo contralateral. Puede haber hidrocele reactivo y engrosamiento del escroto. Diagnóstico por ecografía Doppler: se puede conservar la primera vascularización. Luego se observa un registro de alta impedancia con inversión diastólica y pérdida de flujo sanguíneo. Además, su tratamiento es la cirugía de urgencia. Se presenta el reporte de caso clínico de un paciente pediátrico de 11 años el cual refiere dolor testicular derecho, intenso de 5 horas de evolución. El mismo que se maneja de carácter urgente y se identifica patología escrotal aguda mediante signos y síntomas y exámenes de laboratorio.

Abstract
Testicular torsion is a urological emergency. Although it can occur at any age, it is more common in newborns and adolescents. There are two types of testicular torsion: extravaginal (more common in newborns) and intravaginal (in adolescents). Inadequate fixation of the testicles can cause twisting of the spermatic cord that contains the vas deferens, which can lead to further infarction of the gland. Testicular torsion can occur as a result of injury or exercise, but it can also occur spontaneously. It presents with moderate to severe pain and nausea. On examination, the scrotal lift reflex usually disappears. Subsequently, it is revealed: increased size and echogenicity of the testicles, the testicle may be elevated, and lateral compared to the contralateral testicle. There may be reactive hydrocele and thickening of the scrotum. Diagnosis by Doppler ultrasound: the first vascularization can be preserved. A high impedance recording with diastolic inversion and loss of blood flow is then observed. Furthermore, its treatment is emergency surgery. The clinical case report of an 11-year-old pediatric patient is presented who reports intense right testicular pain lasting 5 hours. The same one that is managed urgently, and acute scrotal pathology is identified through signs and symptoms and laboratory tests.
