Tratamiento quirúrgico y reconstructivo de hemangiomas y malformaciones vasculares extensas en niños: integración de cirugía general, plástica y dermatología pediátrica

Resumen

Los hemangiomas infantiles y las malformaciones vasculares representan anomalías de comportamiento biológico distinto. Los hemangiomas son tumores vasculares benignos proliferativos que aparecen típicamente en las primeras semanas de vida y pueden involucionar espontáneamente, mientras que las malformaciones vasculares son anomalías congénitas de vasos que crecen proporcionalmente con el niño y no involucionan (5). Las lesiones extensas pueden producir complicaciones significativas: ulceración, sangrado, dolor, insuficiencia orgánica o desfiguración. En la última década, el manejo de estas lesiones ha evolucionado con terapias médicas efectivas (p. ej., propranolol para hemangiomas) que han reducido la necesidad de cirugía (4). No obstante, persisten indicaciones claras de tratamiento quirúrgico y reconstructivo, ya sea para resolver complicaciones agudas (obstrucción visual o de vía aérea, ulceración resistente) o para corregir secuelas estéticas y funcionales residuales tras la involución tumoral (3)(4). Las malformaciones vasculares requieren un abordaje individualizado y a menudo multidisciplinario que combine cirugía general y plástica, dermatología pediátrica e intervenciones mínimamente invasivas (escleroterapia, embolización), apoyado en nuevos tratamientos sistémicos como sirolimus en casos seleccionados (10). Este artículo revisa críticamente la literatura reciente (últimos 10 años) sobre el manejo quirúrgico y reconstructivo de hemangiomas y malformaciones vasculares extensas en niños, comparando la eficacia de las terapias médicas versus quirúrgicas, las estrategias según el tipo de malformación, las complicaciones frecuentes y su manejo, y destacando la importancia de un enfoque multidisciplinario basado en la mejor evidencia clínica disponible.

Abstract

Infantile hemangiomas and vascular malformations are vascular anomalies with distinct biological behavior. Hemangiomas are benign proliferative vascular tumors that typically emerge in the first weeks of life and may involute spontaneously, whereas vascular malformations are congenital vessel anomalies that grow proportionally with the child and do not involute (5). Extensive lesions can cause significant complications: ulceration, bleeding, pain, organ dysfunction or disfigurement. Over the past decade, management has evolved with effective medical therapies (e.g., propranolol for hemangiomas) that have reduced the need for surgery (4). Nevertheless, clear indications for surgical and reconstructive intervention persist—either to address acute complications (visual or airway obstruction, refractory ulceration) or to correct residual cosmetic and functional sequelae after tumor involution (3)(4). This review critically examines recent literature (last 10 years) on surgical and reconstructive management of extensive pediatric hemangiomas and vascular malformations, comparing medical versus surgical therapies, strategies by malformation type, common complications and their management, and emphasizing the importance of a multidisciplinary approach grounded in the best available evidence.
