Carcinoma de células pequeñas de ovario tipo hipercalcémico con diseminación fulminante: Reporte de caso

Resumen

El carcinoma de células pequeñas de ovario tipo hipercalcémico (SCCOHT) es una neoplasia extraordinariamente rara y agresiva que afecta sobre todo a mujeres jóvenes y se asocia con hipercalcemia paraneoplásica y mal pronóstico. El objetivo de este reporte es sintetizar el curso clínico-radiológico y los hallazgos diagnósticos clave en una paciente de 19 años con SCCOHT de evolución fulminante, resaltando señales de alerta para su reconocimiento temprano. La paciente consultó inicialmente por dolor abdominal bajo de breve evolución. Durante la evaluación inicial, la ecografía evidenció una masa abdominopélvica izquierda de componentes sólido-quísticos, probablemente de origen ovárico, cuya caracterización detallada se vio limitada por el gran volumen tumoral y por la extensión con compromiso del espacio retroperitoneal. En estudios de imagen de alta resolución se confirmaron ascitis, derrame pleural izquierdo, conglomerados retroperitoneales y diseminación ganglionar masiva pélvica y supradiafragmática, además de metástasis ósea lítica en L2. La biopsia quirúrgica del tumor y ganglios mostró carcinoma de células pequeñas; la inmunohistoquímica evidenció pérdida de SMARCA4 (BRG1) con positividad epitelial focal, confirmando SCCOHT. Debido a la rápida progresión clínica y a decisión de la paciente, se tramitó salida voluntaria; no se inició tratamiento oncológico y el desenlace fue fatal en su domicilio antes de cumplirse un mes desde el inicio de los síntomas. Este caso subraya la agresividad extrema del SCCOHT y la necesidad de considerarlo ante masas ováricas atípicas en adolescentes o adultas jóvenes, especialmente si coexisten hipercalcemia, diseminación linfática extensa y afectación ósea. El reconocimiento precoz, la confirmación inmunohistoquímica (pérdida de SMARCA4) y la planificación multidisciplinaria intensiva son cruciales, mientras se desarrollan estrategias terapéuticas más efectivas.

Abstract

Small cell carcinoma of the ovary, hypercalcemic type (SCCOHT), is an exceptionally rare and aggressive neoplasm that predominantly affects young women and is associated with paraneoplastic hypercalcemia and poor outcomes. The objective of this case report is to synthesize the clinical–radiologic course and diagnostic hallmarks in a 19-year-old with fulminant SCCOHT, highlighting early red flags for recognition. The patient initially presented with short-duration lower abdominal pain. During the initial assessment, ultrasound revealed a large left abdominopelvic mass with solid–cystic components, most consistent with ovarian origin; precise characterization was limited by the tumor’s considerable size and extension into the retroperitoneal space. Cross-sectional imaging subsequently confirmed ascites, a left pleural effusion, bulky retroperitoneal conglomerates with massive nodal spread to pelvic and supradiaphragmatic stations, and a lytic osseous metastasis in L2. Surgical biopsies of the tumor and lymph nodes disclosed small cell carcinoma; immunohistochemistry demonstrated loss of SMARCA4 (BRG1) with focal epithelial marker positivity, confirming SCCOHT. Owing to rapid clinical decline and the patient’s decision, she was discharged home on request; no oncologic treatment was initiated, and death occurred at home before one month from symptom onset. This case underscores SCCOHT’s extreme aggressiveness and the need to consider it in atypical ovarian masses in adolescents or young adults—especially when hypercalcemia, extensive lymphatic dissemination, and osseous involvement coexist. Early recognition, immunohistochemical confirmation (SMARCA4 loss), and intensive multidisciplinary planning are crucial while more effective therapies are developed.
