Fascitis necrotizante proximal de antebrazo derecho: Resolución quirúrgica con reconstrucción mediante colgajo DIEP. Reporte de caso

Resumen
Introducción: La fascitis necrotizante (FN) es una infección rápidamente progresiva de tejidos blandos que compromete la fascia superficial y el tejido subcutáneo, con alta morbimortalidad si no se diagnostica y trata de forma precoz. La cobertura cutánea oportuna tras el control del foco resulta determinante para preservar función. Caso clínico: Mujer joven con fascitis necrotizante del antebrazo derecho de inicio brusco posterior a una puerta de entrada cutánea menor. Se realizó reanimación, antibióticos de amplio espectro y desbridamientos seriados hasta obtener un lecho vital. La reconstrucción definitiva se efectuó mediante colgajo libre DIEP (Deep Inferior Epigastric Perforator) con anastomosis microvascular a vasos receptores del antebrazo, logrando cobertura estable y preservación funcional. Evolucionó sin recidiva infecciosa y con recuperación satisfactoria de la movilidad. Conclusiones: La FN exige diagnóstico clínico temprano, desbridamiento agresivo y antibióticos empíricos adecuados. En defectos extensos del antebrazo, el colgajo DIEP aporta volumen y piel de calidad con baja morbilidad del sitio donante, siendo una alternativa válida.

Abstract
Introduction: Necrotizing fasciitis (NF) is a rapidly progressive soft tissue infection that compromises the superficial fascia and subcutaneous tissue, with high morbidity and mortality if not diagnosed and treated early. Timely skin coverage after controlling the source of infection is crucial for preserving function. Clinical case: Young woman with necrotizing fasciitis of the right forearm with sudden onset following a minor skin entry point. Resuscitation, broad-spectrum antibiotics, and serial debridements were performed until a viable bed was obtained. Definitive reconstruction was performed using a free DIEP (Deep Inferior Epigastric Perforator) flap with microvascular anastomosis to recipient vessels in the forearm, achieving stable coverage and functional preservation. The patient progressed without infectious recurrence and with satisfactory recovery of mobility. Conclusions: NF requires early clinical diagnosis, aggressive debridement, and appropriate empirical antibiotics. In extensive forearm defects, the DIEP flap provides volume and quality skin with low donor site morbidity, making it a valid alternative.
