Depresión mayor y riesgo cardiovascular: ¿Marcador, mediador o factor causal?

Resumen

La depresión mayor es un trastorno frecuente a nivel global, asociado con una alta carga de discapacidad, y a menudo coexiste con enfermedades cardiovasculares (ECV) (1,2). Evidencias epidemiológicas recientes sugieren que la depresión se vincula con un aumento modesto pero consistente del riesgo de eventos cardiovasculares (infarto de miocardio y accidente cerebrovascular) y de mortalidad cardiovascular, incluso tras ajustar los factores de riesgo tradicionales; metaanálisis han estimado un incremento relativo de ~20–30% en el riesgo de cardiopatía coronaria incidente y hasta ~40% en eventos cardiovasculares totales (3,5,9,10). Grandes estudios multicéntricos han confirmado esta asociación: por ejemplo, en 21 países (PURE) los síntomas depresivos se asociaron con mayor incidencia de ECV (razón de riesgo ajustada, HR: hazard ratio, ≈1.14) y mortalidad (HR ≈1.17) a 7 años (4), mientras que en dos cohortes chinas (~539.000 adultos) la depresión se vinculó con aumento significativo de mortalidad cardiovascular (HR 1.22–1.32) y mortalidad por todas las causas (~1.3), con un efecto más marcado en hombres que en mujeres (6). En pacientes con ECV establecida, la depresión predice peor pronóstico (p. ej., ~60–80% mayor riesgo de mortalidad a 2–5 años post-infarto frente a pacientes sin depresión) (13). Vías plausibles incluyen inflamación crónica de bajo grado, disfunción autonómica (reducción de la variabilidad de la frecuencia cardíaca), disfunción endotelial y conductas poco saludables (tabaquismo, sedentarismo, mala adherencia terapéutica) asociadas a depresión. Persiste el debate sobre si la depresión es solo un marcador de riesgo o si contribuye causalmente al desarrollo de ECV; la evidencia sugiere un rol mixto. Análisis genéticos mediante aleatorización mendeliana (Mendelian randomization) apoyan un efecto causal pequeño pero significativo de la predisposición a depresión sobre cardiopatía isquémica (razón de momios, OR: odds ratio, ~1.15), sin evidenciar causalidad inversa (3). Sin embargo, ensayos clínicos de tratamiento de la depresión en pacientes cardiovasculares no han mostrado reducciones claras de eventos, lo que resalta la complejidad multifactorial. En conjunto, la depresión mayor puede considerarse un factor de riesgo cardiovascular emergente, de magnitud moderada, relevante para una evaluación y manejo integrales, aunque se requiere investigación adicional para precisar mecanismos causales e intervenciones efectivas.

Abstract

Major depression is a common disorder worldwide, associated with substantial disability burden, and it frequently coexists with cardiovascular disease (CVD) (1,2). Recent epidemiologic evidence indicates that depression is linked to a modest yet consistent increase in the risk of cardiovascular events (myocardial infarction and stroke) and cardiovascular mortality, even after adjustment for traditional risk factors; meta-analyses have estimated a relative increase of ~20–30% in incident coronary heart disease risk and up to ~40% in total cardiovascular events among individuals with depression (3,5,9,10). Large multicountry studies support this association: for instance, across 21 countries (PURE), depressive symptoms were associated with higher CVD incidence (adjusted hazard ratio, HR, ≈1.14) and mortality (HR ≈1.17) over 7 years of follow-up (4), while two large Chinese cohorts (~539,000 adults) showed that depression was associated with higher cardiovascular mortality (HR 1.22–1.32) and all-cause mortality (~1.3), with a more pronounced effect in men than in women (6). In patients with established CVD, depression predicts worse prognosis (e.g., ~60–80% higher mortality risk at 2–5 years after myocardial infarction compared with non-depressed patients) (13). Plausible pathways include chronic low-grade inflammation, autonomic dysfunction (reduced heart rate variability), endothelial dysfunction, and adverse health behaviors (smoking, physical inactivity, poor medication adherence) that commonly accompany depression. Whether depression acts merely as a risk marker or contributes causally to CVD remains debated; current evidence suggests a mixed role. Genetic approaches using Mendelian randomization support a small but significant causal effect of genetic liability to depression on ischemic heart disease (odds ratio, OR, ~1.15), with no clear evidence of reverse causality (3). Nevertheless, clinical trials treating depression in cardiovascular populations have not demonstrated clear reductions in hard cardiovascular endpoints, underscoring the multifactorial nature of the relationship. Overall, major depression can be considered an emerging cardiovascular risk factor of moderate magnitude, relevant for comprehensive risk assessment and management, while further research is needed to clarify causal mechanisms and identify effective interventions that translate into improved cardiovascular outcomes.
