Resumen

Introducción: La identificación de los factores de riesgo para la recurrencia de la osteomielitis maxilar es un desafío complejo debido a la heterogeneidad de la enfermedad y la variabilidad en los factores predisponentes. Comorbilidades como diabetes, cáncer y enfermedades renales, así como factores locales como infecciones odontogénicas y tratamientos previos, se han asociado con un mayor riesgo. Además, la resistencia a los antibióticos, el uso de medicamentos inmunosupresores y la radiación complican aún más el manejo de esta patología. La falta de consenso en los enfoques clínicos y la variabilidad en la duración del tratamiento dificultan la evaluación de los resultados a largo plazo. Objetivos: Evaluar de manera preliminar la evidencia científica reciente sobre los factores que predisponen a la recurrencia de la osteomielitis de los maxilares y los enfoques clínicos empleados para prevenirla y tratarla. Materiales y métodos: se realizó una revisión sistemática de la literatura disponible en bases de datos como PubMed, Science Direct y el buscador de Google Scholar. Se seleccionaron los artículos que cumplían con los criterios de inclusión-exclusión prefijados. Se eligieron 7 artículos en la revisión final. Conclusiones: La osteomielitis maxilar recurrente es una infección crónica del hueso maxilar causada principalmente por bacterias y hongos. Factores como traumatismos, extracciones dentales, diabetes, y COVID-19 aumentan el riesgo. La formación de biopelículas dificulta el tratamiento. El enfoque clínico combina cirugía y antibióticos, pero la recurrencia es común. La mandíbula y el maxilar superior pueden ser afectados en dependencia del agente infeccioso.

Abstract

Introduction: Identifying risk factors for recurrence of maxillary osteomyelitis is a complex challenge due to the heterogeneity of the disease and variability in predisposing factors. Comorbidities such as diabetes, cancer, and renal disease, as well as local factors such as odontogenic infections and previous treatments, have been associated with increased risk. In addition, antibiotic resistance, the use of immunosuppressive drugs and radiation further complicate the management of this pathology. Lack of consensus in clinical approaches and variability in treatment duration make it difficult to assess long-term outcomes. Objectives: To preliminarily evaluate recent scientific evidence on factors predisposing to recurrence of osteomyelitis of the jaws and the clinical approaches used to prevent and treat it. Materials and methods: a systematic review of the literature available in databases such as PubMed, Science Direct and the Google Scholar search engine was carried out. Articles that met the preset inclusion-exclusion criteria were selected. Seven articles were chosen in the final review. Conclusions: Recurrent maxillary osteomyelitis is a chronic infection of the maxillary bone caused mainly by bacteria and fungi. Factors such as trauma, dental extractions, diabetes, and COVID-19 increase the risk. Biofilm formation makes treatment difficult. The clinical approach combines surgery and antibiotics, but recurrence is common. The mandible and maxilla may be affected depending on the infectious agent.
